Formal Written Complaint
This form is to be used by KHMA members for the purpose of submitting a formal written complaint regarding the behaviour and/or actions of another KMHA member. Upon completing this form, your complaint will automatically be submitted for review to the Abuse and Harassment Director. A copy of your complaint will be emailed to you at the address you provide below. You will be contacted within 72hrs of submitting this complaint.
