This form is to be filled out by any KMHA team requesting permission to raise funds under the Kawartha Coyotes name. Please submit your request well in advance of your proposed event. This application needs to be approved by the KMHA board. The KMHA board meets every 2nd Monday of each month.

Once submitted it is reviewed by the the KMHA board.

Once reviewed the Fundraising Director will contact you to either approve, reject or ask for more information.
